The EGADZ Transitional Outreach program provides intensive support and mentorship to youth who are ready to take the steps and support for preparing themselves for independence.
The Transition Outreach will work collaboratively with various community supports and resources in order to provide the youth with opportunities to gather the skills and knowledge they will need for independent living, maintain their progress on their predetermined outcomes, and connect each youth to their community.
- Referrals can be through an agency or self-referral.
